The Trustee is required to ensure that a member has the most up to date information about the insurance cover available from the Fund at the time a member makes a decision about insurance.

The Trustee has provided the latest information about insurance in the Member Booklet Supplement: Insurance (Employer Sponsored Members).

The Application for insurance or to increase your existing insurance benefits (FSS 014) form can be found towards the end of Supplement under the forms sections.
