Application to reduce or cancel insurance cover

The Trustee is required to ensure that a member has the most up to date information about the insurance cover available from the Fund at the time a member makes a decision about insurance.

The Trustee has provided the latest information about insurance in the Member Booklet Supplement: Insurance (Employer Sponsored Members).

The Application to reduce or cancel insurance cover (FSS 016) form can be found towards the end of Supplement under the forms sections.
