How to join
Personal division
Complete the Personal division member application. You will need to ask your employer to contribute to First State
Super. Complete the Standard Choice form and return it to your employer.
- Download the application form found in the Product Disclosure Statement and return to us
- Download the Standard Choice Form (this includes the Complying Super Fund notice that your employer will need)
and return to your employer
Automatic joining
If you start with a new employer who is a First State Super employer, you will be automatically joined once we receive
the first contribution from your employer.

Retirement Income Stream or Transition to Retirement Income Stream
Complete the Member application superannuation income stream.
FSS Trustee Corporation is the issuer of the Personal Division, the Employer Sponsored Division, the Retirement
Income Stream and Transition to Retirement Income Stream. Before making a decision about any of these products,
