Don't miss out on reports on how your super is performing. First State Super can only keep you informed if your details are up to date.
1. Update your address
It's important to keep your address up to date with First State Super. That way, we can send you information about one of your most valuable assets - your super.
What do to
- Update your address online, or
- Download the form here PDF 150kb, complete and send it to us
- Write to First State Super at PO Box 1229, Wollongong NSW 2500, noting your old address, your new address and your membership number (see your last benefit statement for your membership number), or
- Send First State Super an email noting your old address, your new address and your membership number (see your last benefit statement for your membership number).
If you do not have your membership number please provide us with your:
- Full name
- Date of birth
- Address
- Last employer's name
2. Let First State Super know if you have changed your name.
This makes it easier to keep in touch with you.
What to do
Send First State Super at PO Box 1229 Wollongong NSW 2500 your membership number and a certified* copy of:
- your marriage certificate, or
- Deed Poll evidencing your change of name, or
- change of name certificate issued by the Registry of Births, Deaths and Marriages.
*Certified means that you will need to have a copy of your document signed by a solicitor or Justice of the Peace as a true copy of the original.
3. Provide your Tax File Number.
You don’t have to provide your Tax File Number (TFN) to First State Super. However, if First State Super doesn’t have your TFN:
- You may pay more PAYG tax than you need to when a benefit is paid to you
- It will be more difficult for you to find your super if we lose contact with you
What to do
- Download the form (PDF100kb), complete it and send it to
First State Super, PO Box 1229, Wollongong NSW 2500 or
- Provide your TFN over the phone by calling Customer Service on
1300 650 873 and follow the prompts.
Importance of Tax File Numbers
The Federal Government introduced new measures from 1 July 2007 where members have not provided their Tax File Number to their fund.
If no Tax File Number is held by the Fund

Concessional (employer) contributions eg. superannuation guarantee, other employer and salary sacrifice contributions
|
Non-concessional (after-tax) contributions eg. personal and spouse contributions
|
- If the members account was established before 1 July 2007, amounts above $1,000 will attract tax at the top marginal rate, or
- The account was established on or after 1 July 2007, all amounts will attract tax at the top marginal rate.

- Cannot be accepted by First State Super
