Before you are able to download the Personal Division application form you must download the Personal Division Product Disclosure Statement (PDS) (PDF 1632kb) dated 1 September 2008. Please consider the PDS before making a decision about your superannuation. You will need to be connected to a printer to print and you will also need Adobe Acrobat (click to download).
