Opening a First State Super income stream account is easy

Before you are able to download the Income Stream membership application form you must download the Member Booklet Product Disclosure Statement: Superannuation Income Streams (PDS) (PDF 2477kb) dated 1 July 2009. Please consider the PDS before making a decision about your superannuation.

You will need to be connected to a printer to print and you will also need Adobe Reader which is free to download (click to download).
